Register your team

By registering for this tournament, I acknowledge that participation in volleyball and related activities involves risks of injury. I voluntarily assume these risks for my own participation and agree to release Oki Athletics, Platinum Volleyball, its gym and facility owners, and tournament staff from liability for injuries, loss, or damages arising from my participation, to the extent permitted by law. As team captain, I understand and agree that: - Every player on my team must individually read and sign the tournament's liability waiver and rules agreement on the day of the tournament before participating. - I am responsible for informing all my teammates of this requirement before the tournament. - Players who have not completed the required waiver and rules agreement will not be permitted to participate.

A minimum of four registered teams is required for this tournament to take place. If the minimum is not met and Oki Athletics cancels the tournament, all registered teams will receive a full refund of their $300 registration fee to their original payment method.
